Health Declaration
In order to receive your participant kit you must sign the health declaration.
If someone other than yourself collects the your kit for you, he / she must submit: a photocopy of your ID card/passport, your health declaration form signed by you, which includes authorization letter from you, as a condition for receiving your kit.
